This will never survive constitutional challenges in countries like the United States and Canada in which it is specifically stated the government cannot force this on its citizens and therefore the signing of any document that contravenes that right would be deemed illegal. In Canada, health is also a provincial jurisdiction and not a federal one, and in the United States, I also believe the states have complete jurisdiction to decide on these matters.
