Good catch. There's definately no needle. The way the nurse immediately starts pulling the syringe upward makes that very clear. The syringe is moving upward before the needle would even be out of the arm. @Cobia1 You can frame by frame on NewTube. Look for the "+1" at the bottom. One frame, the 'needle' is in the arm, the next, the syringe is out about a centimeter and up about a centimeter. If there were a needle on that syringe, it would be pulling her skin upward.
