You are seeing only optical and lighting differences.
The April picture has the camera slightly off to Fraudi's left side and the lighting is very broad frontal lighting cause sharp lighting and shadows. His mouth is closed causing his head to appear more ovoid.
Picture two in July has the camera slightly to Fraudci's right. The lighting looks to be a mix of window light to his left and room light in general causing softer lines and shadows effecting the appearance of his nose and eye. He is also in a speaking expression in this pic with mouth slightly open, chin/jaw elongated changing the appearance of the shape of his head. This also causes his ears to slightly pull in to a bit and in combination of the camera off to his right and the different lighting.
Try it to yourself. Look at your ears in mirror with mouth closed and the start opening your mouth and you can see and sense the tug on your ears by your jaw moving down.
These two pics are of the same POS💩 asshole evil corrupted man that I have not seen a patient in 40 years Dr. Fraudci is.
I actually think it's more of a lens thing. The left one looks like a typical portrait, so something like full frame + 80mm or small tele. The right one seems to be from something akin to laptop camera, something wider. On a wide lens (with short focal length), from relatively close distance, the middle of the face would look to be "inflated", like on the right photo.
