It may be mandatory to report deaths that occur immediately after somebody gets a shot, but doctors rarely connect deaths from vaccines that happened days or weeks after the person gets a shot so they are not being reported in or not mandatory under those circumstances. In fact their predilection is to not associate such events with the vaccine unless it happens immediately upon getting the shot.
